2 Ag are systematically performed. 1 or more Ag requested: 1 single 5 ml tube. Sample must come with completed "Immuno-hematology" clinical information form. Mention the comprehensive identity of the patient (birth name, spouse name, first name(s), date of birth, sex), the date and time of collection, the name of the prescribing physician and of the collecting person. Attach the phenotype group if known (otherwise, performed and invoiced in addition). Identitity must be strictly identical on the tube and on the test request form.
