Sample must come with completed "Immuno-hematology" clinical information form. Mention the comprehensive identity of the patient (birth name, spouse name, first name(s), date of birth, sex), the date and time of collection, the name of the prescribing physician and of the collecting person. Identitity must be strictly identical on the tube and on the test request form.
